Job Summary

Colorado PACE has an immediate opening for a Care Plan Coordinator.

Location: Aurora CO

Status: Part-time

Schedule: Monday-Friday 24-28 hours per week

Hourly Pay Range: $25.29 - $29.71

Supplemental Pay:Based on position schedule and/or availability:Overtime

Mileage and Expense Reimbursement:Sixty-two and a half cents per mile one of the highest in the industry!

REWARDING WORK YOU WILL DO:

  • IDT Meeting Coordination: Manages the schedule logistics and documentation for all initial assessments periodic reassessments (at least every 180 days changes in services or as participant condition requires) and annual care plan reviews of participants by the IDT.
  • Documentation: Accurately documents the IDT consensus goals and prescribed services into the participants electronic health record (EHR) ensuring the final care plan is approved and signed by the appropriate IDT members and the participant/caregiver.
  • Regulatory Adherence: Ensures all care planning documentation timelines and content meet the stringent standards set by the Centers for Medicare & Medicaid Services (CMS) and the Colorado Department of Health Care Policy & Financing (HCPF) for PACE program.
  • Monitoring Provision of Services: Tracks and monitors the provision of care as prescribed on the approved care plan to ensure adherence to required time frames. Identifies areas of risk and non-compliance to the Director of Quality & Compliance and supports with coordinating appeals processes where applicable.
  • Liaison: Serves as the primary administrative point of contact for the IDT members regarding care plan implementation and changes.
  • Service Implementation: Communicates the final updated Plan of Care to relevant PACE staff (e.g. Home Care Coordinator Transportation Staff Adult Day Center staff etc.) and contracted providers to ensure services are initiated promptly and accurately.
  • Participant Communication: Schedules and coordinates meetings between the participant/caregiver and the IDT ensuring participants are actively involved in the development and revision of their care plan per PACE requirements.
  • Identifying Updates to Care Plan: Collaborates with the IDT (Interdisciplinary Team) to foster a culture of vigilance providing the guidance and mentorship necessary to recognize triggers for care plan revisions and ensures participant needs are met as quickly are the participants needs require.
  • Ensures compliance with appropriate regulations and standards of care. Participates in survey processes.
  • Ensures compliance with policies and procedures.
  • Data Integrity: Maintains accurate and up-to-date data within the electronic medical record system pertinent to plans of care and provision of services.
  • Reporting: Assists the Director of Quality and Compliance with generating reports related to care plan timeliness utilization patterns and compliance metrics for Quality Improvement (QI) initiatives.
  • PACE/CMS Regulations: Strong working knowledge of PACE model requirements CMS regulations (42 CFR Part 460) and state-specific regulations for Colorado (10 CCR 2505-10 8.497).
  • Technical Proficiency: High proficiency in electronic health records (EHR) systems and Microsoft Office Suite (Word Excel Outlook).
  • Organizational and Analytical Skills: Exceptional ability to manage complex schedules track multiple timelines and ensure accurate detailed documentation.
  • Interpersonal Skills: Excellent verbal and written communication skills for effective collaboration with the IDT members and compassionate interaction with participants.
  • Completes annual mandatory education requirements.
  • Collaborates with peers to move Colorado PACE toward its Quality and Compliance goals.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Minimum Education: Associates degree in healthcare administration or a related field required. LPN/Bachelors degree preferred.
  • Minimum Experience: One or more years of experience in care coordination case management or a highly administrative role within a healthcare setting (PACE managed care home health or long-term care preferred)
  • Required License: Valid Colorado Drivers License and proof of auto insurance.

PHYSICAL REQUIREMENTS:

  • Ability to lift/carry a minimum of 30 lbs
