• 3 Years Experience: $115000 $127587

• 5 Years Experience: $127587 $140000

Shift Details: Days (within a 7:00 AM 10:00 PM window weekdays only) with potential on-call rotations or occasional extended hours.

Position Overview

Navitas Healthcare LLC is seeking a dynamic and results-driven Director of Case Management to provide strategic and operational leadership for hospital-based case management services. This role is responsible for overseeing departmental performance driving care coordination excellence and ensuring effective discharge planning processes that promote high-quality patient outcomes and efficient resource utilization.

The ideal candidate will demonstrate strong executive presence collaborate effectively with interdisciplinary teams and lead initiatives that align clinical operational and financial objectives.

Key Responsibilities

Department Leadership

• Direct and evaluate all case management operations including staffing models skill mix FTE management and workflow optimization.

• Implement and leverage information systems and technology to enhance departmental efficiency.

Performance & Quality Improvement

• Analyze departmental metrics and drive sustainable performance improvement initiatives.

• Ensure compliance with regulatory standards and internal quality benchmarks.

Resource Management

• Optimize staffing and productivity to meet operational and quality goals.

• Oversee utilization management care coordination and discharge planning functions.

Care Coordination & Compliance

• Ensure timely and accurate documentation and adherence to established care coordination processes.

• Promote seamless transitions of care across departments and external agencies.

Executive Collaboration

• Partner with executive leadership to meet financial performance and growth objectives.

• Build strong working relationships with medical staff nursing leadership legal and compliance teams.

Qualifications

Education:

• Bachelors degree required

• Masters degree (MSN MHA or MBA) preferred

Licensure:

• Active RN LCSW or LMSW license required

Experience:

• Minimum 3 years of experience in acute care hospital case management

• Minimum 2 years of leadership experience in acute care case management

Skills & Competencies:

• Strong communication and presentation skills

• Proven ability to drive results and achieve measurable goals

• Demonstrated leadership in interdisciplinary collaboration
