Position Summary: Under general supervision the Family Care Coordinator is responsible for coordinating care between medical substance abuse and mental health services. This position is subject to working evenings and weekends. No smoking workplace.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Helps families to navigate the largest health systems.
- Assists in SBIRT Services and FASD assessment.
- Provides MI for health-related issues.
- Provides outreach and engagement.
- Participates in scheduled trainingand meetings as necessary.
- Participates in program data collection and submission as necessary to meet project goals.
- Maintains records and completes appropriate documentation.
Education/Experience:
High School diploma or general education degree (GED); or one to three years experience or training in substance abuse treatment or social service field; or equivalent combination of education and experience. Annual completion of Blue Basin training in the following courses: Annual completion of thefollowing Blue Basin training: Understanding of Motivational Enhancement Therapy; trauma abuse and neglect violence Post-Traumatic Stress Disorder (PTSD) and related conditions or other approved equivalent training. Must be familiar with Medicaid TANF and Childrens Health Insurance (CHIP) eligibility.
Certificates and Licenses:
Licensed Master Social Worker (LMSW); if driving on SMH business must have a valid Texas Drivers License and provide proof of current insurance.
Approval Requirements:
Position requires approval from TDCJ and HCCSCD.
