We are seeking a full-timeTransitional Healthcare Facilitator for needs at Pendleton Juvenile Correctional FacilityinPendleton Indiana.

Available Shift: full-time days Monday-Friday

Pay Rate $25 per hour benefits!

The Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished and those re-entering society are provided with necessary services and resources. Short term travel will be required to visit other juvenile facilities within the state of Indiana.

Qualifications

Bachelors degree in psychology social work counseling or related field from an accredited institution required
Two (2) years of experience working in a correctional setting
Experience with/knowledge of community and state programs and services
Two (2) years of experience in re-entry services or release planning preferred
Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening
Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use
Must be able to pass background investigation and obtain agency security clearance where applicable
