Location: National Park Medical Center - 1911 Malvern Ave Hot Springs AR 71901
Shift: PRN M-F 0800-1700

Were seeking a Case Manager to guide patients and families through the rehabilitation journey ensuring seamless communication effective care planning and smooth transitions.

How youll contribute:
Lead interdisciplinary care coordination and discharge planning
Identify barriers to progress and help develop solutions with the care team
Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
Facilitate weekly team conferences and family meetings
Communicate patient progress and discharge plans to caregivers
Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
Support additional departmental needs as assigned
Qualifications and requirements:
Active licensure as an LVN RN or Social Worker
Bachelors Degree; preferred
2 years of case management or social work experience in an acute or rehabilitation setting; strongly preferred
Strong communication organization and collaboration skills
Proficiency in Microsoft Office
Case Management or Rehab Nursing certification (CCM ACM RN BC ARN); preferred
