Job Summary

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West Great South Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics Primary Health Care Counselling NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

SWAMS is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. About the role

The Community Connector Access plays a vital role in supporting Aboriginal and Torres Strait Islander people to understand and access the National Disability Insurance Scheme (NDIS). This position focuses on building trust raising awareness and providing culturally appropriate information to individuals families and carers about disability supports available through the NDIS.

Working closely with community members local organisations and service providers the Connector helps identify eligible participants supports them through the application process and ensures they feel confident navigating the system. This role is ideal for someone who is community-minded culturally aware and passionate about improving access to disability services in remote areas

Selection Criteria:

  • Demonstrated experience in advocating and supporting others to reach goals or outcomes
  • Ability to support community members in reaching their goals-whether through encouragement guidance or helping them access services.
  • Ability to prioritise tasks and follow up particularly in busy environments
  • Able to manage day-to-day responsibilities by staying organised following through on commitments and keeping track of what needs to be done.
  • Verbal and written communication
  • Able to communicate clearly and confidently in both writing and conversation using persuasive communication to influence inform and build understanding.
  • Strong understanding of and experience working in disability services in particular NDIS services Awareness of disability-related challenges and a willingness to learn about NDIS supports with any lived or community experience considered valuable.
  • Demonstrated ability to build trust and rapport with individuals and groups to achieve positive outcomes for them
  • Skilled at connecting with people listening respectfully and building relationships that help others feel safe heard and supported.
