- Provide direct patient care on a Med Surg Tele unit delivering evidence-based nursing interventions to adult populations with diverse diagnoses.
- Monitor telemetry vital signs and clinical status; recognize early signs of deterioration and escalate promptly.
- Administer medications manage IV therapies assessments wound care and infusion therapies as prescribed.
- Lead discharge planning patient and family education and coordination with case management to ensure smooth transitions.
- Collaborate with physicians pharmacists therapists and ancillary teams to optimize care plans.
