Human Touch Home Health is seeking a Licensed Master Social Worker (MSW) for a PRN home health position serving patients in the Fredericksburg Virginia area.

What Youll Do
Conduct comprehensive psychosocial assessments in the home setting
Develop and implement social work care plans
Provide counseling and emotional support to patients and families
Assist with discharge planning and long-term care arrangements
Connect patients to community resources and support services
Participate in interdisciplinary case conferences
Ensure compliance with Medicare documentation standards
What Were Looking For
Active Virginia Master Social Worker (MSW) license required
Masters degree in Social Work
Minimum 1 year healthcare experience (home health preferred)
Strong knowledge of community resources and discharge planning
Excellent communication and documentation skills
Valid drivers license & reliable transportation
