Summary:
The Social Care Assessor is a vital non-qualified role within Liverpool City Councils Adult Social Care and Health directorate. You will contribute to the operational delivery of an effective Adult Social Care Service by undertaking assessments and support planning for adults with care and support needs.The core of this role is to apply the Prevent Reduce Delay principles of the Care Act 2014 ensuring that residents receive the right support at the right time to maintain their independence and wellbeing. You will work as part of an integrated team often serving as the first point of contact to provide information advice and strengths-based interventions.
Responsibilities:
- Conduct assessments that focus on an individuals personal relational and community strengths rather than just their deficits.
- Act as a care navigator to help individuals access appropriate services from health voluntary and private sectors.
- Manage a caseload of adults with social care needs ensuring timely intervention and high-quality service delivery.
- Identify and escalate potential safeguarding concerns or high-risk situations to Senior Social Workers or Team Managers.
- Maintain accurate sensitive and up-to-date digital records on systems such as Liquid Logic ensuring compliance with Data Protection and Council policies.
Qualifications:
- Literacy numeracy and basic IT skills.
- NVQ Level 3 or 4 in Health and Social Care or a relevant vocational qualification (e.g. CAVA). Desirable
Essentials:
- Experience working with people/service users in a health social care education or advice and guidance environment.
- A working knowledge of the Care Act 2014 Mental Capacity Act and Safeguarding Adults procedures.
- An understanding of personal budgets and the importance of achieving best value for the Council and the resident.
