What Youll Do:
Develop and implement individualized MSW plans of care in collaboration with physicians and the interdisciplinary team
Assess psychosocial needs and determine eligibility for home health services
Provide counseling resource coordination and crisis intervention
Re-evaluate and adjust care plans as patient needs change
Support patients and families navigating complex healthcare decisions
What Were Looking For:
Masters Degree in Social Work (required)
Minimum one year MSW experience within the last three years
Home health or hospice experience preferred
Experience with geriatric population preferred
Flexibility to meet patient scheduling needs including new admissions
