Main Function:

• Conduct Medicare Annual Wellness Visits in accordance with CMS guidelines under provider supervision
• Collect and document comprehensive patient histories including:

• Medical surgical family and social history
• Medication reconciliation (prescription OTC supplements)
• Allergies and immunization status

• Perform and document required screenings:

• Depression screening (PHQ-2/PHQ-9)
• Cognitive impairment screening
• Fall risk assessment
• Health risk assessment (HRA)
• Tobacco alcohol and substance use screening

• Record vital signs height weight BMI and other relevant clinical data
• Identify preventive care gaps and assist with development of personalized prevention plans
• Provide patient education on preventive services chronic disease management and lifestyle modifications
• Ensure accurate and timely documentation in the electronic health record (EHR)
• Assist providers with outpatient clinical workflows as needed
• Prepare patients for provider visits and procedures
• Administer injections immunizations and medications per scope of practice and provider orders
• Perform basic clinical procedures consistent with LPN licensure
• Coordinate referrals diagnostic testing and follow-up appointments
• Communicate test results and care instructions to patients per protocol
• Maintain exam room cleanliness and stock supplies
• Adhere to infection control safety and HIPAA regulations
• Support quality initiatives population health programs and preventive care metrics
• Collaborate with providers care coordinators and administrative staff
• Demonstrate compassionate patient-centered care and professionalism
• Additional responsibilities required
• Floating to other GFP offices may be required

Education:

• Current unrestricted Licensed Practical Nurse (LPN) license (state-specific)

• Graduation from an accredited practical nursing program
• Knowledge of outpatient clinical workflows
• Basic computer skills and experience with EHR systems
• Strong communication organizational and documentation skills

Experience:

• Experience conducting Medicare Annual Wellness Visits

• Familiarity with CMS preventive care guidelines
• Outpatient primary care or internal medicine experience
• Bilingual skills preferred
